Notice control

CMS WISeR outcomes

Record what the official notice says, not what an internal label predicts or a staff member paraphrases.

Reviewed July 24, 2026 • CMS guide v7.0, updated July 24, 2026

CMS and official participant notices control. WISeRly is independent, not CMS-affiliated or endorsed, not an official WISeR participant, does not issue determinations, and does not guarantee authorization or payment.

Keep outcome concepts separate

Preparation result

An internal indication of gaps or readiness. It has no official status and predicts no result.

Official review outcome

The determination or status communicated through the assigned participant’s official channel. Preserve the exact language and conditions.

Claim payment outcome

A separate downstream event subject to all applicable claim, coverage, coding, and Medicare requirements.

Outcome handling workflow

  1. Authenticate and retain the official notice under organizational policy.
  2. Record date received, service/case linkage, exact status, rationale, conditions, and deadlines.
  3. Route clinical questions to qualified clinicians and billing/legal questions to appropriate staff.
  4. Follow only the resubmission, reconsideration, claim, or other route stated in current official instructions.
  5. Close internal tasks only after the official next step is documented.
Enhanced technology or AI-assisted review can support document analysis, but the human clinical determination remains distinct. WISeRly cannot convert a software score into affirmation or non-affirmation.
