CMS controls. Verify current Arizona applicability, participant assignment, selected services, codes, and submission instructions with CMS and the assigned official participant. WISeRly is independent, not CMS-affiliated or endorsed, not an official WISeR participant, does not issue determinations, and does not guarantee authorization or payment.

Arizona readiness sequence

1. Verify the location basis

Do not rely on a mailing address alone. Match the facts required by current official instructions, including where the selected service will be furnished.

2. Confirm current scope

Check beneficiary program, service and code, date, place of service, and any exclusion or exemption before selecting a review path.

3. Follow the assigned channel

Use only the official participant’s current method for submission, status, requests for information, and notices.

Prepare without predicting

Organize the order, relevant history, objective findings, prior conservative treatment where required, and service-specific support. Preserve signatures, dates, page order, and provenance. A complete-looking packet can still receive an unfavorable determination; readiness is not approval.

Human determination: enhanced technology or AI-assisted review may support an official workflow, but it is separate from human clinical determinations. WISeRly’s preparation checks are neither.

Check applicability factors · Use the documentation checklist · Plan official handoff

Official sources

Public WISeRly resources and demos use synthetic information only. Never submit patient names, identifiers, clinical records, claim data, or other PHI.
